



Accessing Your Enrollment Forms
To begin the enrollment process for a contract, click on the “Unenrolled Contracts” section of your “My Action Items” on the My Contracts tab or on the Dashboard tab. Any contracts that need the enrollment forms completed will appear in the list under the category heading. Click on a contract from the list to open the enrollment pages for the contract.
You can also click on the “Enrollment” link under the “Contract Activities” section in the contract overview to open the enrollment pages.

Incomplete Paperwork Status
Once you begin completing the enrollment paperwork, your contract status will change from “Assigned” to “Incomplete Paperwork”. A green check will appear next to each page on the left menu once you complete all required information and move to the next screen.

Helpful Features
There are several helpful features to assist you while completing the enrollment paperwork for your contract:
The contact information for your administrator can be found on the right of the header at the top of the enrollment pages. Click the arrow next to the “Contact My Admin” section to view your administrator’s contact information.
The information you enter is autosaved when you click out of a field to prevent you from having to reenter information.
You can exit your enrollment forms at any point using the “Save and Exit” button on the bottom left of each enrollment page. All your information will be saved, and you can pick up where you left off when you return.
